How can I increase or change my coverage?

To request a change in coverage due to a life event, such as a separation, please complete the Notice of Change in Coverage form available from your plan administrator and give it back to him/her once completed.

It is important to submit the completed form within 31 days of the request. Changes received after 31 days may require evidence of insurability and approval by the insurer.  

Please keep a copy of your completed forms for your records.
