My family situation has changed. How can I add or remove a family member?

To request a change in coverage further to a life event, such as a separation, please complete the Coverage Change form available from your plan administrator and give it back to him/her once completed.

It is important to submit the completed form within 31 days of the request. Changes received after 31 days may require evidence of insurability and approval by the insurer.
