How can I submit a treatment plan/estimate?

To submit a treatment plan or an estimate, simply forward all the information through your Members portal by going to the Submit a claim tab.

Once your request has been forwarded, AGA will analyze it and may pre-authorize the expense based on your contract coverage. A written confirmation of the decision will be posted on your portal.
