How to read your drug/dental Telus card?

A. The pharmacist must enter 35, while the dentist must enter 610226. This is your group plan provider number. In this case, it identifies AGA FINANCIAL GROUP INC.

 

B. This sequence of six digits is your group number (001850 in the example). When entering the number into the system, the pharmacist must always start by entering two zeros (00) followed by the four digits of your group. 

 

C. This is your certificate number made up of the following characters:

a.    The first character is a zero;
b.    The next 4 characters are the first 4 letters of your last name*;
c.    Followed by the first 2 letters of your first name; 
d.    Ending with 3 digits.



Below is an example for a member named Gilles Boivin:

0BOIVGI005 
In this ID number, the 6 underlined characters are letters, while the others are digits.


* However, should your surname include less than 4 letters, each missing letter will be replaced by a zero (0). Below is an example for a member named Martin Roy:

0ROY0MA259
In this ID number, the 5 underlined characters are letters, while the others are digits. 

Your group and certificate numbers are also displayed on your wallet card that you will find on the Members portal, under the My group insurance tab. These numbers are also featured on the claim statements and the welcome email.
